Once your application has been completed, you can schedule your appointment online and should provide your confirmation number in the comments section or email it to ame@carefirstdocs.com. If not received at least 24 hours prior to your scheduled appointment, airman may be subject to a $35 penalty. Valid photo identification also should be brought to your appointment as well as any supporting documentation like a special issuance, laboratory test results, physician office visit notes, etc.
